At Wonderful Wellness, we worked to combat the high prevalence of chronic diseases affecting our company’s employee population. I managed a panel of 103 patients diagnosed with Type I and Type II Diabetes Mellitus. As a Health Coach, I utilized individualized goal setting and Motivational Interviewing techniques to help patients understand the multiple dimensions of health and to provide disease self-management support. My primary responsibility was to coordinate the complex care of these patients across medical specialty lines (including Nutrition, Behavioral Health, and Physical Therapy) while also performing phlebotomy and pharmacy dispensing.
